Diabetes Educational and Professional Materials Order Form

To receive free educational and professional materials from the Florida Diabetes Prevention & Control Program please print, complete and mail, fax or email back to us the order form (see link below). Shipping is the only cost for requestors. Many materials are available in Spanish. The Diabetes Health and Care Diary is the only item available in Haitian Creole at this time. Be sure to send both pages of the form. Orders take approximately two weeks to fill.
